I think, fundamentally, the problem is that 'Human' Rights are based on a fairly narrow understanding of what it is to be human; or, to put it slightly differently, mental impairment challenges our understandings of the human condition - are these people still human? Are they something less, something different, or just part of the natural variation?

There is also an overlap with how we treat non humans, and the issues we have with that.
