I think this is one case where you should definitely be using her preferred pronouns. She thought she was a woman right up until her condition was discovered. No, she shouldn't be allowed to compete in female athletics, but you don't have to humiliate her too.

It's just a pronoun. And you are muddying the waters. This case is different to that of Lia Thomas.
